Donor Siblings: How Many Is Too Many?

There is no single agreed number. National rules run from one child per donor in some countries to around a dozen families in others, sperm banks set their own international caps, and donations arranged privately are counted by nobody. In July 2026 ESHRE proposed a European limit of 50 families, falling later to 15.

That disagreement is the same wherever you live, which is why this guide covers it once for everyone. The question of how many donor siblings a child ends up with is partly a matter of arithmetic, partly a matter of what large sibling groups do to the people in them, and only partly a matter of law. The legal detail differs by country and belongs on the country pages for the United States and the United Kingdom.

What is a donor sibling?

A donor sibling is a person conceived with sperm or eggs from the same donor, in a different family. Genetically they are half-siblings, sharing about 25 percent of their DNA. Some people use the informal word “dibling”. The relationship carries no legal status anywhere, and it exists whether or not anyone knows about it.

Donor siblings are distinct from the children a donor raises himself, who are also genetic half-siblings to everyone in the group. That matters when people count. A donor with four children of his own who has helped ten families has produced a genetic sibling group larger than the family count suggests.

How many donor siblings can one donor have?

In practice the number depends on three things stacked on top of each other: what the country of treatment allows, what the bank or clinic allows, and how carefully anyone counts. The largest documented groups run into the hundreds. Sperm from Jonathan Meijer, a Dutch donor who began in 2007, was used to conceive between 550 and 600 children according to the Dutch court case brought by the donor-conceived foundation Stichting Donorkind, which ended with an order in 2023 that he stop donating.

Groups of that size are outliers rather than the norm. The Donor Sibling Registry, a US nonprofit that has connected more than 27,000 people with half-siblings or biological parents since 2000, reports half-sibling groups it has matched that run to around 250 people. Most people who go looking find a handful. The point is that nobody can promise a small number in advance, because the counting happens after the fact.

Why do the limits differ so much from country to country?

Because each country picked a different thing to count, and picked it for a different reason. Data presented at the ESHRE annual meeting in London in July 2026 showed that Malta and Cyprus allow a donor to contribute to the birth of a single child, while other countries set limits by families rather than children, so that a family can have full genetic siblings without using up the quota twice.

Family-based limits are now the more common design, and ESHRE recommends them explicitly. Denmark, one of the largest exporters of donor sperm in the world, sets a national limit of 12 families. The rules that apply to a reader in the United States or the United Kingdom are different again, and those sit on the US and UK country pages rather than here, because getting them slightly wrong in a shared guide would be worse than not writing them at all.

What did ESHRE actually propose in 2026?

The European Society of Human Reproduction and Embryology published a position paper in Human Reproduction recommending an EU-wide limit on the number of families per gamete donor, starting at 50 families and reducing over time to a maximum of 15 families or lower, supported by an EU-wide donor registry to monitor compliance.

The paper adds several conditions that get less attention than the headline number. National limits stay in force where they are lower. Donors may set their own personal limit below the cap. Gametes should not be distributed to new families more than 20 years after a donor’s first donation. Banks outside the EU that export into the EU would be covered. Families are counted as potential live births unless confirmed otherwise, which closes the gap where a shipped vial is assumed to have produced nothing.

ESHRE is a professional society rather than a legislature, so the position paper carries the weight of a recommendation to clinics, banks and regulators. The paper was developed by an expert working group and revised after a stakeholder review in which 45 organizations took part, including groups representing donor-conceived people, patients, national fertility societies and gamete banks.

Why do limits exist at all?

Three separate arguments, and they do not point to the same number. The first is the chance that two donor siblings meet and form a relationship without knowing they are related. The second is that a genetic variant a donor carries reaches many children at once. The third is what a very large sibling group does to a person’s sense of who they are.

The consanguinity argument is the oldest and the most quantified. ASRM’s guidance for the United States repeats a long-standing suggestion that in a population of 800,000, limiting a single donor to no more than 25 births would avoid any significant increased risk of inadvertent consanguineous conception, while noting that the figure depends on the population base and the geographic spread of the samples. That reasoning treats donor siblings as a statistical problem inside a fixed population, which is exactly the assumption that international shipping broke.

ESHRE takes the third argument most seriously and is honest about the evidence. Its paper states that the evidence base on the psychosocial impact of large donor sibling groups is small and developing, that definitive evidence of harm is limited, and that its recommendations therefore take a precautionary approach. The proposed 15-family figure is flagged for review as knowledge develops. Nobody in this debate is claiming to know the right number.

What happens when a genetic variant reaches a large sibling group?

It becomes very hard to contain, and the case that changed the European conversation shows why. A donor at the European Sperm Bank in Denmark was found to carry a rare TP53 variant, associated with Li-Fraumeni syndrome, which raises lifetime cancer risk substantially. His samples had already been used across many countries. By May 2025, 23 children had been confirmed to carry the variant and 10 of those had been diagnosed with cancer. Reporting in December 2025 put the number of children fathered by the donor at 197 or more, across 14 countries.

The bank’s own published account of the case is worth reading carefully, because it explains why screening did not catch this. The variant is newly arisen and previously undescribed, and it exists as a gonadal mosaic, meaning it is present in only a small proportion of the donor’s sperm cells and not in the rest of his body. The donor himself is unaffected, and a standard genetic screen of his blood would not show it. The bank states that not all children conceived with his samples have inherited it, and recommends that all of them be tested.

The European Sperm Bank has since applied an international family limit of 75 families per donor, introduced at the end of 2022, and says it supports binding international limits and an EU registry. It also acknowledges that national limits were exceeded in some countries, in this case and in others, for reasons it describes as primarily technical and administrative. Read plainly, that is a bank saying its own counting failed.

How do people find their donor siblings?

Three routes, and most people who succeed use more than one. Consumer DNA databases such as Ancestry and 23andMe match people who share segments of DNA, and they work whether or not the donor ever agreed to be found. Voluntary registries let families and donors register the same clinic and donor number and match each other directly. Some countries operate a statutory register through which a donor-conceived person can request information at a set age, and what that register releases differs by country, which is a question for the US and UK pages.

The DNA route has quietly overtaken the others, and it is the reason anonymity is no longer something anyone can promise. A donor who donated in 1994 on the understanding that he would never be identified can be identified today by a cousin who took a saliva test for fun. The same mechanism is what turns a sibling group of eight, as recorded by a clinic, into a sibling group of forty in practice.

What do donor sibling relationships look like in practice?

Mostly ordinary, and more varied than either the hopeful or the alarmed version suggests. In a survey of 165 donor offspring recruited through the Donor Sibling Registry, published by Jadva and colleagues in Reproductive BioMedicine Online in 2010, 15 percent were searching for donor siblings only, 13 percent for the donor only, and 64 percent for both. Of those searching for donor siblings, 33 percent had found them, 95 percent of those had been in contact, and half were in contact at least once a month.

Later research describes the difficulties honestly. Connections can be hard to navigate when the people involved want different things from them, when one is far more interested in the donor than the other, or when distance makes contact expensive. Some people describe donor siblings as close family and some describe them as interesting acquaintances, and both descriptions come from inside the same sibling group.

Size changes the character of it. A group of five can hold a group chat and meet. A group of a hundred cannot function as a family in any recognizable sense, and people in very large groups describe the experience in terms of scale rather than kinship. One donor-conceived woman who found 25 half-siblings over seven years told the Guardian in 2024 that it makes you feel “a bit mass-produced”. That sentence is doing more work in the European policy debate than any of the statistics.

How do you find out how many families a donor has already helped?

You ask, and then you consider how much the answer can be checked. A bank or clinic keeps a count and will state its own family limit, and that count is only as good as the reporting from the clinics it ships to. A private donor has whatever record he has kept himself, and no independent register stands behind it. The UK regulator makes the same point about donations arranged outside a licensed clinic: there is no control on the number of times a person can donate or on the number of children born.

This is where we should be plain about our own role. We are a place where people find each other. We do not verify anyone’s donation history, we do not screen anyone’s health, we hold no record of births, and we handle nothing biological. The checks, and the decisions, stay with you. Asking a donor directly how many families he has helped and how many more he plans is a normal part of a first conversation, and how someone answers it tells you a good deal.

The HFEA also reports that some serial donors operating outside clinics have been linked to more than 200 children in the UK, and that some of them push for “natural insemination”, meaning unprotected sex. The regulator states plainly that this is not more successful and that nobody should feel pressured into it. Both patterns tend to appear in the same profiles, which is a reason to treat vague answers about numbers as information rather than as an oversight.

Does a large sibling group affect what you tell a child?

It affects the timing more than the content. Families who tell early have the easier job here, because the existence of donor siblings becomes one more ordinary fact rather than a second disclosure years later. A child who has always known can be told at eight that there are others, and can decide at eighteen whether to look for them.

The part parents find hardest is that the number is not fixed. A group of six at the time of a child’s birth can be a group of thirty by the time that child is twenty, because other families keep taking DNA tests. Saying “we know of six so far, and there may be more” is accurate at every stage, and it avoids a correction later.

What can you decide, and what is out of your hands?

You can decide which route you use, and each route hands you a different amount of control over the number. Buying from a bank means accepting that bank’s family limit and its international distribution, and some banks let you choose donors who have set a lower personal limit. A known donor arrangement means the number depends on one person’s honesty and on his own plans, with nothing enforcing either.

What nobody controls is the future. Limits change, banks merge, records move, and a donor who intended to stop at three families may be asked repeatedly to help a fourth. A guarantee of a small sibling group is beyond anyone’s reach, so the realistic question is how comfortable you are with an unknown that will resolve itself over your child’s lifetime rather than yours.

How many donor siblings does the average donor-conceived person have?

There is no published average, because no country holds a complete count and cross-border shipping means no national register would be complete anyway. What is documented is the range: some donors contribute to a single family, and the largest known groups run to several hundred. ESHRE's 2026 position paper describes exactly this gap and proposes an EU-wide registry as the mechanism that would make an average knowable.

Is a family limit the same as a limit on children?

No. A family limit counts recipient families, so a family that has three children with the same donor uses one place in the quota. A child limit counts births. ESHRE recommends family-based limits so that siblings within a family can be full genetic siblings without the quota penalizing them, which means a limit of 15 families can produce considerably more than 15 children.

Can a donor find out how many children he has?

It depends entirely on where he donated and under what system, and in some places the number is treated as protected personal data. The European Sperm Bank, for example, states that it does not disclose the total number of children conceived using a specific donor because Danish authorities consider that figure to be the donor's personal data under GDPR, and that it can only release it where legally required. Donors who arranged donations privately have only their own records.

What is the risk that two donor siblings meet by accident?

It is low in an open population and rises with concentration. ASRM's guidance repeats the suggestion that in a population of 800,000, no more than 25 births from one donor avoids a significant increase in the risk of inadvertent consanguineous conception, while noting that the figure needs modification when the recipients form an isolated subgroup or when samples are distributed over a wide area. Both of those exceptions describe the current market rather well.

Do sibling limits apply to donations arranged privately?

No register exists for them, which is the honest answer everywhere. Clinic and bank limits bind clinics and banks. The HFEA states that people donating outside a licensed clinic fall outside its remit and that there is no control on the number of times they donate or the number of children born. Whatever number a private donor gives you is a statement about himself, and the only verification available is consistency over time.

Should I ask a bank how many families a donor has already reached?

Yes, and ask what the limit is, whether it counts families or births, and whether it applies worldwide or only in one country. Those three answers together tell you far more than the current tally, because the tally will keep rising while the samples remain in distribution. Some banks also offer donors with lower personal limits, which is worth asking about before choosing.

Does a large sibling group harm children?

The research does not show that, and it is not strong enough to show the opposite either. ESHRE's position paper states that the evidence base on the psychosocial impact of large donor sibling groups is small and developing and that definitive evidence of harm is limited, which is why it describes its own recommendation as precautionary. The practical version is short. Ask what the limit is, ask whether it counts families or births, ask whether it is national or worldwide, and ask what happens to the count when samples cross a border. With a private donor, ask how many families he has helped and how many more he plans, then ask again in a few months and see whether the answer holds together.
